Q. How do I get pre-authorization for home oxygen? A- Home oxygen is considered Durable Medical Equipment (DME). When your doctor feels you may require the use of home oxygen he should contact the nurse case manager with the request. Documentation required from your doctor is a Certificate of Medical Necessity, the prescription, all office notes to support the need for oxygen, the latest CXR (chest x-ray) or CT scan reports, PFT (Pulmonary Function Test) reports for the last 6 months and Arterial Blood Gas results if done. All documentation must be received and reviewed before home oxygen will be approved. A second opinion by a Board Certified Pulmonologist may be required before authorization can be given. Q. How do I arrange for a second opinion appointment if required? A. If you receive a letter requiring a second opinion for any DME please call the nurse case manager at 1-888-563-1564 ext 127, and she will call the doctor of your choice to arrange the appointment. When the appointment is set up the nurse case manager will call you with the details. Q. How do I find out about a claim that has been denied? A- You may contact us at 1-888-563-1564 and ask for the nurse case manager. It is helpful if you have available the EOB (Explanation of Benefits) you received to refer to when asked by the nurse case manager. Q. If I don't qualify for the Libby Medical Program, are there any other options open to me? A- Yes, you may be able to apply for coverage under the Libby Asbestos Medical Plan (LAMP). Check our Health and Resource Links page for contact information. Please be aware that LAMP is in no way affiliated, connected or adminstered by the Libby Medical Program. Q. If my application is denied, do I need to start over if I reapply at a later date? A- No, if you are denied admission, the LMP will keep your application on file. If your medical condition changes, it is not necessary to fill out a new application. You or your doctor may simply notify the LMP of the changes and request the application be reviewed again.
