You are eligible for coverage under the Libby Medical Program if you are an Eligible Individual and you are diagnosed with an Asbestos-Related Medical Condition or Illness.
You are an Eligible Individual if:
- You worked in the Libby Mine or Mill at anytime, or
- You were the spouse or legal dependent of someone who worked in the Libby Mine or Mill at the same time the worker actually was employed there, or
- You lived or worked within a 20-mile radius of the Libby Mine or Mill for at least 12 consecutive months at anytime before January 1, 2000.

There are a few things that you should know about the application process that have changed since the new Libby Medical Program 005 went into effect on July 20, 2005.
- The medical requirements necessary for an eligible individual to become enrolled in the LMP have been minimized in LMP-005. Now, verification of a single pleural plaque by peer review is all that is required. The peer review process involves the independent review of a chest x-ray or CT scan by a board certified radiologist who is also a chest radiologist, coordinated by HNA/Triveris. In the case of chest x-rays, the peer reviewer will also be a "NIOSH" certified B Reader. ("NIOSH" is the National Institute for Occupational Safety and Health)
- A LMP application may now be signed by any licensed physician. Prior applications required the signature of a board certified pulmonlogist or internist.
- The need to demonstrate impairment on pulmonary function testing is no longer required to enroll in the LMP. (But, remember, benefits are generally still only paid to treat asbestos -related conditions and illnesses)
- If an applicant is denied admission, HNA/Triveris will keep his or her application on file. If the applicant's medical condition changes, it is not necessary to fill out a new application. The applicant or doctor may simply notify HNA/Triveris of the changes and request the application be reviewed again.
